About UDID
The Unique Disability ID (UDID) project is implemented by the Department of Empowerment of Persons with Disabilities (DEPwD) to create a national database of persons with disabilities and issue a unique identification card. The UDID system provides a centralized platform for applying for a Disability Certificate and UDID card. Eligible applicants can register online, complete the application form and submit the required documents. The concerned medical authority reviews the application, carries out the disability assessment and may issue the Disability Certificate and UDID card electronically. The UDID can be used as a common identification document for accessing applicable disability-related government benefits and services.
